Health Insurance Marketplace Statement

If you or anyone in your family enrolled in a health insurance plan through the Health Insurance Marketplace, you will receive Form 1095-A, Health Insurance Marketplace Statement.
You will receive the Marketplace form, and you will need to submit it when you file your tax return with the IRS.
